We collect only the information you choose to provide so an appropriate ministry leader can respond. Access to the secure care dashboard is limited to approved care-team members.
Confidentiality
Pastoral notes and requests should be shared only with the leaders responsible for follow-up. Please do not submit emergency information through this form.
Records and requests
Records should be reviewed regularly and kept only as needed for care, safeguarding, financial, or legal responsibilities. To request a correction or deletion, contact a ministry leader.
